Gather your supplies for making your mask.  I bought this pack of 24 masks at my local arts & crafts store for $3.  Punch out the mask you want and use a sanding block to smooth the edges.

I printed out the poem on cardstock to layer on top of the mask.  You can use any patterned paper you want.  Trace the mask shape on the paper and cut it out.

Using your Tombow Adhesive, adhere the cardstock to the mask.  I always use removable adhesive when adhering paper to paper. It always goes on slightly crooked for me and I need to be able to move it.

Perfect fit!  And the extra cardstock makes the mask really strong so you can add lots of embellishments!!

I wanted to have pretty, swirly, rhinestones on my mask. Prima Say It In Crystals are perfect for this!  They come with adhesive on them but they can still fall off.

Tombow Mono Multi Adhesive is perfect for this!  It has a fine tip for detail work, it goes on white and dries clear. It also has a broad tip. I used it to adhere the rhinestones to my mask.

I also used the Tombow Mono Mulit Adhesive for the feathers and flower. When you are done embellishing the front, curl the mask a little so it fits on your face.

I added ribbons, one to each side so I could tie the mask on and one that I glued flowers to.  The ribbon with the flowers is to hang off the mask while you wear it.

I added some pleated ribbon to the top to finish it off.  I LOVE how it came out and I can't wait to wear it for Halloween!

Here is a close up.  You can add a stick to the side if you don't want to tie it on.  The ribbon is easy to curl and looks pretty if you want to leave it hanging down.  Enjoy making your mask!
