color of yarn i used was "stuyvesant". i tried a plastic hook, but found steel hook worked better. yarn is bulky, multi-ply that separates, holding yarn taunt made it easier to work with.
start: chain 3 then...
slip stitch in end to form ring.
row 1: chain 3 then...
make 9 doubles in ring. slip stitch to connect.
row 2: chain 3, then make double in top of next stitch. chain 1, then double in top of next 2 stitches. continue around and slip stitch to connect.
row 3: chain 3 and double in top of next double, chain 1. skip chain 1 in previous row and double in top of next 2 doubles. continue around and slip stitch to connect.

row 4 thru ?: chain 3, then double in top of each stitch all around. slip stitch to connect. repeat, adding rows until bottle holder is as tall as you would like. (my bottle holder had 10 rows total).
I wanted my holder tall enough to hold my Vera Bradley cup as well.
to make strap: do not tie off yarn. chain 3, then double crochet in top of next 2 stitches. turn, then chain 3, double in top of next stitch and top of chain 3. turn and repeat.
continue until strap is as long as you would like. i made mine 36" long.
fold bag in half to determine placement of other end of strap.
slip stitch strap in place.
knot off yarn and cut leaving 3" tail.
pull end through several loops inside bag to secure.
finished bag.
